The purpose of the HEMS Aftercare Lead is to provide a long-term interface between the HEMS Care Group, Hampshire and Isle of Wight Air Ambulance charity and our patients. The post holder will join an established HEMS service to support patients and their families to understand their complex critical illness or injuries, including their pathway of pre-hospital critical care and anaesthesia
 
A detailed working knowledge and experience in advanced Critical Care is required for this role.  Specifically, the ability to analyse and interpret complex clinical and non-clinical facts in a range of settings in order to meet the ongoing needs of patients and families is crucial.
 
The Aftercare Lead will provide complex emotional support and guidance for patients and families following their pre-hospital critical care experience, along with explaining the Critical Care provided on scene. The post holder will take continuing responsibility for the assessment of care needs, development, implementation and evaluation of patient care, and provide ongoing signposting and referrals. They will demonstrate empathy and highly developed communication skills and the ability to manage and relay sensitive information to families including those receiving life changing news.

The Aftercare Lead will work within regional networks, the wider NHS and relevant charitable trusts to ensure service development and partnership working with organisations who provide ongoing care for patients that HEMS have treated.
 
The post will be based between the Hampshire and Isle of Wight Air Ambulance airbase at Southampton Airport and University Hospital Southampton. The successful candidate will also be required to visit patients in hospitals across Hampshire, the Isle of Wight and surrounding areas too. 

You will be a HCPC registered paramedic with evidence of post graduate study in critical care, along with a full UK driving licence.
