Key Responsibilities:
Clinical Practice
Provide expert diabetes assessment, monitoring, and management to inpatients with diabetes across all wards.
Collaborate with medical, nursing, dietetic, and pharmacy teams to optimise glycaemic control and reduce diabetes-related complications.
Initiate or adjust insulin therapy, oral medications, and glucose monitoring strategies in line with clinical guidelines and patient needs.
Lead on the care of patients with diabetes emergencies (e.g., DKA, HHS, hypoglycaemia).
Act as an advocate for patients, promoting patient-centred care and self-management principles.
Education & Training
Deliver formal and informal training to ward staff on inpatient diabetes management, including safe use of insulin and hypoglycaemia prevention.
Support patient education around diabetes care, treatment changes, and discharge planning.
Leadership & Service Development
Contribute to the planning and delivery of high-quality inpatient diabetes services.
Participate in audits, quality improvement initiatives, and policy development related to diabetes care.
Act as a clinical leader and role model, supporting junior staff and students.
Communication & Documentation
Maintain clear, concise, and accurate patient records in accordance with legal and organisational standards.
Communicate effectively with all members of the MDT and external agencies involved in discharge planning and community follow-up.
