In this pivotal role, you will coordinate complex patient discharges, working closely with multidisciplinary teams, patients, and families to ensure continuity of care beyond hospital walls.

Work collaboratively with clinicians, social workers, and external agencies to streamline discharge pathways.

Advocate for patients by ensuring their preferences and needs are central to discharge decisions.

Identify and resolve barriers to onward care, including housing, care packages, or equipment requirements.
