Job Title:  Complex Discharge Team Leader

Contract Type: Permanent 

This is an exciting opportunity to join our team based within the Acute Discharge Hub at UHS.  We are inviting Nurses, Physiotherapists and Occupational Therapists with leadership skills, patient-centred care and a passion to improve pathways and services within acute care to join us.

 You will be responsible for a small team which is made up of nurses, AHPs and Discharge officers.  You will be required to work across wards and divisions, link with multiple internal/external agencies, drawing upon your clinical skills to effectively support communication for our most complex patients.  We are a vibrant and friendly team here to assist the wider multi-professional teams whilst placing the patient at the heart of health and social care networks.

The successful candidate will be responsible for managing the teams undertaking the assessment and discharge planning of patients within the defined clinical area, working under the direction of their Complex Discharge Operational Lead. In addition, act as a case manager using their clinical knowledge and experience to support the day to day operational functions of the teams, completing their own assessments and interventions where indicated.

You will be part of the senior leadership team offering teaching, supervision and operational direction, supporting the drive for quality and timely assessment to safely discharge patients.

You will be expected to be a confident communicator with excellent organisational skills. You will be required to liaise with family members, patients, and members of the Multi-Disciplinary Team on a regular basis and be competent with IT systems that facilitate this. You will need to demonstrate an ability to work in a busy and often pressurised environment, be able to provide highly specialised clinical discharge planning advice both within the Trust and externally, to other team members, in support of wider health and social care professionals.
